I also give my consent and authorisaon to Worldwide to deploy the Qurban sacrifice to its partners worldwide and to make any decision, in the best interest of achieving the intended purpose of Qurban.

Saya ______ wakilkan *Qurban/Aqiqah/Nazar/Qurban Hajat/Qurban Syifa’ *saya/pihak _ kepada Worldwide dengan membeli *seekor kambing/lembu/sebahagian lembu di negeri ______. Serta menyembelihnya dan membahagikan *sepenuh/sebahagian dagingnya kepada yang memerlukan. Saya memberi keizinan kepada Worldwide dan pembekal (supplier) untuk mengambil keputusan yang terbaik demi memastikan ibadah ini dijalankan dengan sempurna.
